At InnerWorks Counselling London, our Case Management Services are designed to help individuals and families reduce stress, increase access to care, and feel more supported through life’s challenges. We provide comprehensive support including:

Who is case management for

Our Case Management Services are ideal for individuals and families who feel overwhelmed trying to navigate complex systems of care. You might benefit from case management if you:

  • Are caring for a neurodiverse child, teen, or adult and need help accessing funding, services, or school supports

  • Are an adult with a disability or mental health concern trying to understand what programs and services are available

  • Feel stuck or frustrated filling out forms for ODSP, DSO, housing, or other support programs

  • Are trying to coordinate care across multiple providers and need someone to help connect the dots

  • Want advocacy in meetings with schools, service providers, or healthcare professionals

  • Are facing a major life transition, such as aging out of youth services or moving into adult care

  • Just don’t know where to start and need a knowledgeable guide on your side

What to Expect:

When you reach out for Case Management at InnerWorks Counselling London, you’ll be connected with a Bachelor of Social Work (BSW, RSW) or Registered Psychotherapist (qualifying) who understands how to navigate complex systems and advocate effectively on your behalf.

Here’s what the process looks like:

  • Initial Intake: Your case manager will meet with you to understand your situation, identify your goals, and determine what types of support are most needed.

  • Consent & Collaboration: We’ll review and sign any necessary consent forms to allow us to advocate and coordinate with schools, agencies, or healthcare providers on your behalf.

  • Personalized Support Plan: Together, we’ll map out next steps—whether that’s applying for funding, requesting services, or connecting with community supports.

  • Ongoing Help: Your case manager can work with you during scheduled sessions, and also in between sessions to complete paperwork, make phone calls, and liaise with service providers.

  • Advocacy & Navigation: We support you in meetings, help cut through red tape, and ensure your voice is heard when it matters most.

Case Management is billed at $100 / session and is available on a flexible basis to meet your needs—whether that’s a one-time consultation or ongoing coordination and support.
